academic inactive

I'm going academic inactive next semester since I'll be student teaching 5 days a week, which is basically like the real world!  I was told that theres a form online in member services that I need to fill out, but no one is being proactive and telling me where to find it, so I figured I would ask here.

Anyone have any ideas?

that's what I thought, from being on EC in the past, but they're telling me I need to fill out some forms?! I sent in my letter saying why I was applying for it and thought that that was all I needed to do.

when i went inactive Debi sent me the form. I don't think i have it anymore on my computer, but i think i still have the email. I'll try to find it for you.
