I think Jack especially acted out of character in places, but especially in that episode. He takes his team into risky situations, yes, but he is seldom so cavalier about their safety.
And don't get me started on Ianto! This is the man that hijacks a JCB to rescue his concrete encased lover and *still* finds time to don a hard hat, a hi-vis vest and (correct me if I'm wrong - I can't bear to watch it again) a pair of big flipping boots! Please, please don't expect me to believe he would go in that room without any protective gear, because I can't. He has always been depicted as efficient and capable, would he really change so radically?

He totally was wearing huge boots! I know because I rewound it and laughed at them. ;-)
Not to mention, they're freaking Torchwood and you'd think they'd have some kind of protocols in place for dealing with hostile aliens - inside aquarium tanks or not. I mean, I know Jack's style can be kind of situational and take-it-as-it-comes, but still! Ianto was trained by Torchwood One - you'd better believe they would have planned for it.
