do you mean an icon or in the body of a post?

For an icon you go to manage in the top toolbar and then Userpics, then upload userpic, it needs to be 100x100 pixels though.

In the body of a post: you need to have the picture hosted on a website like photobucket or something. then you copy the picture information and put it in the html tag

It should say " in the html tag <".img.scr = "photo info here"./> " except without the period points.
