In what context are you asking? I believe many universities administer uniform student rating measures across all classes & instructors. Are you wanting advice on devising such measures for a department or something? (Sorry if I am missing the thrust of your question.)

The biggest problem is that student evaluations of teachers are almost entirely worthless.

I hand out a fresh baked cookie with each individual student's initials carefully lettered in frosting on eval day.
