I say explain to the teacher what is going on and say you will continue to try and get it done in a timely manner, you don't expect an extension but you aren't blowing off the class.

Don't give up hope, and don't let your mom give up hope either. There are alternative centers, check UTAH and Arizona for alterntive treatments. Meanwhile, ask for an extension --teachers can be understanding.
