Kind of valentine art, but not really, and belated.

Warning: Mild gore I guess because of...organ.

I guess the pic is pretty much open to interpretation. It could be just Snape wicked sense of humor: some poor animal heart, to mock tradition. Or it could be Snape's idea of an adequate valentine: precious dragon heart to be used in many potent potions.
But it my mind it's pretty clear that it is Snape own heart he ripped from his chest himself. Maybe because Harry left him and he couldn't cope. Or maybe is it the ultimate proof of his loyalty to The Cause, an ingredient to be used in a ritual to vainquish Voldemort ('heart of your allies freely given...'). In any case I think it would be a fitting method for a potions master to put an end to his life, I just can picture him methodically harvesting useful parts from his own body to preserve them if he decide to kill himself for whatever reason.
Couldn't bring myself to look for proper references for the heart (*shudders*) so I'm aware it is kind of joke-gore. Don't have excuse for the parcel wrapping though, just lazy.
