Ok, first an apparition (demonic probably) and then the lady, Gloria, is in a mental institution and Sam appears there. She has stabbed a man in the heart because God told her to.

Sam clearly lost some faith when Father Gregory's spirit appeared. He so wanted it to be an angel, because he wanted to believe so badly. Then it turned out to be just another spirit.
When the spirit looked at Sam and said: "Some people need redemption, don't they Sam." you could see that Sam silently agreed with his words. He sees himself as in need of redemption, with his "destiny" and with all that's happened in his life.
Poor Father Gregory, he looked so bewildered and disappointed when he was told that he's not an angel like he believed he was.
Dean, on the other hand, gained some new faith by having seen concrete evidence of fate at work with his own eyes. I loved the uncertainty in his voice when he said he might have seen God's will when he saw how the bad guy was killed.
The job is taking it's toll on the brothers and they both have their own ways of coping with it. Dean continues as the sceptic and Sam as the believer, but this episode showed their viewpoints getting a bit closer to each other.
