Looks to me like if you die on the island, you stay dead (like the Dharma folks), but if you die somewhere else, you might be seen on the island. Why did Richard look like he never aged? Perhaps because he's dead...but then how did he meet Juliet in Florida and Portland?
Did you notice that Ben was born outside Portland? 23 miles away, in fact.
Any significance to "54439" the code Ben used on the fence? (or was it "54339"? I have to double-check) And how did he get it, anyway?
