[The camera bounces for a moment as it, and the rest of Itachi's things, are thrown to the side in a haste.  He's on route 31, still fairly close to the city, but he's sitting on the side of the small lake next to the road.  His hands are dipped into the water and he's frantically scrubbing them clean.]

[Itachi isn't the only one who's seeing the people he's killed.

...Kisame is just snarling at them empty space instead of doing the hand-washing thing. He looks dead tired.]

You're dead, you liar. You were selling our secrets to Konoha!

[And then he glances in another direction and flinches.]

I had to. You would have killed me to graduate too--

[usepokeforehead]

[Itachi looks a bit startled at Kisame's voice and accusation. He's not aware that Kisame is speaking to his own hallucinations.]

What...?

[usedgigadrain]

[Itachi's voice actually makes him focus. He starts a little and his eyes clear.]

Itachi-san....

[He grabs his head with a groan.] Something's happening to us-- Or are you lying to me too, like all the rest?

[...somebody is a little paranoid]

[usepokeforehead]

And what exactly would I be lying to you about, Kisame?

[He stops washing his hands but they remain in the water, shaking just slightly. Has Kisame finally realized the truth about him or is it something else?]

My entire life has been a lie; why should you be different?

You know genjutsu is my weakest point--why are you doing this?

[Because seeing dead people obviously = genjutsu and Itachi is the best genjutsu user he knows]

[He looks away, a distant look in his eyes. He has to try and keep calm or else things can very easily fall apart.]

You know just as well as I do that our chakra is sealed in this world. I could not have cast a genjutsu even if I wanted to.

I murdered my year-mate over 22 years ago, Itachi! He's standing right there; what else could it be?

[He's pointing at empty air. But interestingly, just made an unusual choice of words.]

What reason would I have to torment you with visions from your past?

[He's staring straight ahead now. Now, along with the blood, family members are starting to appear in his peripheral vision. They're simply staring at him with an accusing glare, but Itachi can still feel their gazes on him. It's taking all his willpower to focus on Kisame.]

[He moves abruptly like he's dodging something before he speaks again. Itachi does have a point...]

Then who is?

[usepokeforehead]

I don't know. I don't know how it is even possible.

It has to be something like genjutsu--

[Because the alternative is actually scaring him.]

...Your hands are raw, Itachi-san.

[Combating hallucinations attempt one: focus very hard on something else.]

I have been attempting to scrub the blood off of them for some time now. I have not had much success.

[Focus on Itachi's hands. Focus]

I don't see any blood. You too...?

Yes...it would seem that I am suffering from similar illusions.

[He lifts his hands from the water, in his mind both water and blood dripping from them. Could this really be all in his mind? Rarely did he ever get caught in an illusion of any sort.]

[Itachi caught by genjutsu... that is seriously disturbing.]

With our chakra sealed...how do we combat this?

[usepokeforehead]

I am...not sure.

[He opens his mouth as if to speak again but his words die in his throat as another hallucination appears, this time from the water in front of him. Kisame cannot see it, but Itachi and clearly see his dead best friend emerging from the water in front of him. Shisui, skin tinged blue and with bruises adoring his neck, approaches and Itachi goes from calm and collected to clearly frightened.]
