Shockingly, the Nanny is doing a terrible job

I know the nannies are stupid and all, but out of the blue, they will only get bottles for the toddlers over and over. No diaper changes, won't put them in the crib, just nannies stuck on endless loop pulling bottles out of the fridge.

Is this a known conflict with having more than one nanny at once, or custom NPCs at all? I didn't see anything in past entries.
