From my background in a school district in San Antonio, absolutely. The kids have to sign forms. At least the kids have to sign permission forms where I'm from. And get this, I'm a college student who took an Art History class two semesters ago. My professor had to inform us in the syllabus that the work we were going to cover would contain nudity. Why? Because not everyone was above 17 of age. (Not all colleges are like this. I attended RISD for awhile and as you can imagine, we all knew what to expect in a school like that XD).
Even if it's not considered controversial it only takes one parent to shout 'lawsuit!' The school district has to take extra measures in preventing this from happening, however tedious the task may be. A teacher could get sued just for pointing at a student (yes, ridiculous but that's 'zero tolerance' for you) so I can't imagine a class going to a field trip without signed permission forms and have the potential of upsetting a student.

a side note, the one time I've seen maile full frontal nudity on network tv was the simpsons episode. they briefly showed a statue's boy bits.
