Harry still wasn't used to getting birthday presents. He was surprised anyone remembered, surprised anyone bothered, and surprised when each year he got more than the last.
This year, he was surprised by the small, black gift, caught up in the pile presented by Hedwig and now almost hidden under discarded paper.
He was more surprised to recognise the script inside the card. "Harry," it read, nothing more, but even a teenage boy will recognise the hand which scrawls "Another abysmal failure" on all of one's papers.
When he learnt of his "private lessons" and the relevance of the gift, his surprise turned to cynical disdain, and he scoffed at the irony of the timing.

Harry was surprised the following birthday by a box, no card attached.
He was more surprised there was no purpose to this gift, nor apparent hidden agenda.

He was no longer surprised at the timing, the third year in a row. He was surprised Severus had remembered, surprised he knew at all.

Now his birthday's nearly over and his children are asleep, and Harry sneaks the small black box out of the paper, no card.
He's no longer surprised to find this, and will be surprised when they end. But as long as there's a small black box hidden at the bottom of the pile, he'll have a happy birthday.
