massage therapy verbal intake

Depending on where you work at, I feel that your verbal intake should be different.  Obviously in a medical setting, there are charts/paperwork that you can rely on to get better information to help assess your client.  But really there is just no substitute to a good few minutes of asking the client/patient what is going on with them.

Here is my standard intake when it comes to working at a spa.  Very simple, clean and it gets to the point.

**Have you ever had a professional massage before?  (If they answer yes, move to A, if no, move to B)

A:  That's great, and were they good experiences for you?

B:  Ok... you will be disrobed and you will be laying down completely covered.  Is this comfortable for you?

**Is there any specific areas you want me to concentrate my work on?

**Is your skin sensative to any types of oils/creams/lotions?

**Is there any areas you don't want me to work on?

**Do you have any questions for me?

**If at anytime, you need me to change my pressure, please let me know.

That's it.. pure and simple.  Covers everything.  The more you do that, the more you will realize that people will just open up to you and tell you everything that you need to know.
