Alex is too sensitive - he totally shouldn't have flipped out over the Meredith thing. Poor Alex.
Hospital ranking = EXCELLENT PLOT - although I really would have been amused if PPTH edged them out (everyone there is so competent).

Shonda has no idea how to do Callie/Erica. Although having both of them being awkward is an interesting twist - usually one is and one isn't. And usually someone has an inkling that they're at least into the same sex.
But the whole "almost virgins" thing was wrong. I mean yeah, the dynamic can be different to being with a man, but it isn't necessarily. I mean it's still another person with complex emotions - what happens with the genitals isn't what it's necessarily all about. Perhaps if they'd never been in love before...but mostly the dynamic was believable, just the "I don't like kissing girls" and "we're almost virgins" was a really stupid way to write it.
The not liking to kiss girls comment made Callie sound like she'd gone and sucked face with every woman in the hospital to make sure it was only Erica she liked kissing. And the almost virgins comment implies that there's some rule that says you are still a virgin to women if you have only slept with men.
I also don't buy Erica being just as inexperienced as Callie - she's always seemed way less skittish about the whole thing, like she's biding her time, especially when she admitted to Addison that Callie was beautiful. Plus ZOMG I'M STRAIGHT freakout is irritating at the best of times, and when it's doubled it's worse. But yeah, I don't think Erica is the type to flip about that sort of thing the way Callie would.
Poor Lexi. I'm really liking her and George - if nothing else I love seeing him having a true friend whose issues he doesn't have to deal with every five seconds.
And the Cristina icicle? *DIES* Her poor interns, I love how they took a picture of her. I think she really realized that they need to learn though, which is good.
I cannot wait to see what the Chief instigates to get the program back on track. If it's House-esque I will laugh SO. FUCKING. HARD. I can so imagine House selling off his hiring program. But I think the Chief is going to get really tough on the teaching, which should be great - since people will actually have to try.
And Izzie? So sad. She was in the background so much this episode, but had presence all the same. I think her patient let her truly let go of Denny - she let herself have a little fantasy and realized that the pain wasn't as raw as before and maybe she could let go. I think she's really going to become a butterfly this season - she's come to a point where she can't go back to George or Denny or maybe even Alex. The change is a subtle one, but I think it's there.
Ultimately, I think Alex is running from his feelings for her. I think he's confused because he let her in and he's realizing that maybe it's because he feels something for her. But he's not ready to have anyone know him that intimately so he's pushing her away. I've never really shipped Izzie & Alex, but I can't wait to see how he deals with/heals after the whole Ava thing. I think he might be the one to crack under the pressure this year, and perhaps it'll make Izzie realize what she feels or something.
