I'll argue that, instead of appearances, more boys and girls have low self esteem based on grades and societal expectations of education. From personal experience, more of my teenage misery was caused by lack of achievement in school and not how I looked.
In my longer comment, I mentioned eating disorders which are a real problem but are hardly average (estimated less than 3%). 1/5 of Americans have mental illnesses which means the majority are not eating related. Besides, the study say that it's an actual condition in their brains and not something they can control which goes to say they are naturally inclined to have the disorder, not that society gave it to them.
It is not a modern issue because focus on attractiveness has existed throughout history although I do agree that the media has exacerbated the issue. It is more of an issue now, but that doesn't mean it's only modern.
If romance was based on attractiveness according to the media, then the rates of single vs. taken people would be different... but they're not. Fairy tales existed long before television marketing which flooded children's brains with romantic ideals.
I think we define "common person" differently because most of the people I would consider common aren't as deluded victims of society as you described.
