Dean has been a vampire for some time and he thinks he has his bloodlust under control. He finally thinks it safe to get into bed with Sam again.
However while they are getting hot and heavy, Dean loses control and feeds on Sam until the latter goes limp in his arms. Realizing what he has done, he claws his own heart out from his chest.
