In case you were wondering, yes I did forget this thing existed. But I did remember earlier this month and was contemplating what to post. I was thinking of updating you all on the craziness of things that have happened over the last several months in the work life. Yes, there will always be crazy work stories. But honestly some of the things that have happened have been way out there. One of the patients from the low income maternal clinic that we work in asked to transfer care to me after not having seen me in 4 months (I had done her first two visits). I see her one time in mid pregnancy at late second trimester. Then a week later I get a call from the OB intern saying that she had coded at home (code means your heart stops beating). Her and her baby both didn't make it and though final autopsy report is pending the verbal was there was no obvious cause of death. Crazy, I tell you.
Second story is about one of my pain patient was acting strange and had very poor self care (read: he hadn't bathed in a while), which was unusual for him. He says he keeps passing out and thinks he's having seizures so I direct admit him to the hospital from clinic. He leaves the hospital against medical advice ("AMA") and returns to clinic. He is one of my chronic pain patients and demands a script for his pain medication. He, however, now is not making sense. We quickly decide to call security to escort him to the ER. After a tense few moments, they arrive and I warn them that it looked like he had a hammer in his coat. They make him drop his coat and handcuff him. He ends up having a hachet (or some witnesses called it a full sized ax) and a bow knife. And subsequently was found to be high on multiple substances including amphetamines. Crazy story #2.
So to temper all that excitement, I'll given some random encouragements I've received. This newborn baby is added to my schedule and I am perplexed by this. Normally newborn babies are taken on by the doctor that delivered them. As soon as I step in the room, I realize that I recognize the mom and that I take care of her other kids. She didn't say anything as to why she had chosen to follow up with me but I understood from the action that she trusted me with this new baby and appreciated the care given. It was cool.
Another one of my patients said that she had decided to establish care with me because one of my other patients who was her friend had recommended me.
That and my clinic schedule has been ridiculously busy. I've actually had more patients booked that the residents who are a year ahead of me. I also have this patient that the first time she met me she said that I would be her doctor and that she wouldn't have any other doctors. She hadn't seen a doctor in more than 10 years. She ended up subsequently getting diagnosed with two primary types of cancer and having more than a handful of doctors, but still she frequently reminds me that she doesn't want me to leave Tulsa and if I go only slightly far away that she'll follow me. It's cute.
I'm trying to figure out where I am supposed to go next and what the rest of my practice will look like. It's a big decision but in my field there still are lots of opportunities to tailor my practice.
Last month I was on Gynecology and I learned a tremendous amount and did lots of colposcopy. Before that was Orthopedics for a month. Before that was a month of senioring our family medicine service. And in Jan I went on a mission trip to a country far away- but that should definitely be a separate post. Hope the anonymous person asking for an update feels heard. =)
