I finally found a copy to use, so here it is.

CHARACTER NAME:
CHARACTER SERIES:

[OOC]
This is the permissions list for OOC (out of character), activity.
Answer the following questions with "yes" or "no", as well as additional information if desired.

Backtagging: Toooootally fine with that.
Threadhopping: For sure!
Fourthwalling: None of the "you're not a real person"-he won't believe you anyway. But, if your character has met or read about him, feel free to know who he is, quote him, or know things about him as revealed in canon.
Offensive subjects (elaborate): Eeeeeh, that can be sticky. I don't like the overuse of swearing, unless it's IC. If you believe something might be offensive, let me know, and we'll talk about it.

[IC]
This is the permissions list for IC (in-character), activity.
Answer the following questions with "yes" or "no", as well as additional information if desired. With IC permissions, it's a good idea to elaborate on what other players can expect from your character if they choose to do any of the following:

Hugging this character: You can go ahead and hug him! He'll be a little surprised that you'll want to, but it's nice of you.
Kissing this character: Whoa, this is going fast! He'll likely freak out a little bit at a surprise kissy face. If he has feelings about that person, he'll melt a little.
Flirting with this character: You can go ahead. But either he won't get it, or go squeaky on you. More likely the first then the second.
Fighting with this character: That'll have to be set up between us. He's not much of a fighter, though he knows kido(basically, poem spells). But since he realized the lack of medical support, he's going to try to avoid one.
Injuring this character (include limits and severity): I don't mind an injury, but again, let me know how bad. Set up before you go slicing and dicing would be awesome.
Killing this character: I'd rather avoid it if I can. But again, set up first please.
Using telepathy/mind reading abilities on this character: Go ahead! But set it up first with me please. I'd like to know what sort of things should be 'heard' IC

Kinks
yes: I'm pretty open, but I would like to keep it IC. CR as result of sex, vanilla, light bondage, persuasion (if not outright pressure), cross dressing, massages, hugs, hickeys...if you're not sure, pm me!
maybe: hair pulling, bloodplay...pm me if you're not sure!
No: vore, gore, bathroom play. Kinda grosses me out. If you're not sure, just pm!
orientation: he's pretty much bi curious. He hasn't been out and about so much, so he doesn't know if either gender is good or bad for it. He's surprisingly open minded.
Prefered Pairings: Really, while I am fine with the whole shipping idea, I love love love to have pairings that develop over time. There's some weird ones out there, and I love seeing what happens when two characters get to know each other..

Warnings:
None that I can think of. Just that, he has a healing blade (he'll warn you before he uses it) and if he's going to have difficulty healing you, lemmie know please!

Character Abilities/Powers |
Zanpakuto: Hisagomaru (a Katana with a blue handle and a circular guard with a brown sphere or bag attached to it. There is a gauge that goes up the blade, which will be explained below) Power: When the blade touches the flesh of an injury, it is healed. At the same time, the gauge fills with a red gas/energy. Smaller wounds will barely fill the gauge at all, while Encroachment (the process of which a ghost becomes a Hollow) is great enough to instantly fill the gauge. When the meter on his blade fills up, the blade automatically goes into the second mode (Akeiro Hisagomaru, which is smaller, the size of a scalpel), and unleashes all the wounds absorbed onto the target Hanataro designates with a slashing movement. The attack is similar to a Cero (energy blast). After doing this once, Hanataro may attack again, not needing to heal further; however, it would be difficult, due to Akeiro Hisagomaru's small size.

Healer: He is an above average healer, with the ability to restore the spiritual energy, and the ability for the body to function after injury. He uses, just like others of his company, treatments and techniques that could be compared to the human medical world. However, unlike the human world, he is able to heal his patients within a night, even hours of the trauma that has occurred. Endurance: Considerable. He had to heal more than one person from the point of near death in the space of a few days, at least to the point they could defend themselves. As well, he was able to partake in a considerable amount of activity, and was able to keep on going despite only having a few hours sleep and a placebo for energy.
Swordsmanship: Though no expert by any means, he at least has the basics of sword to sword combat, in order to have attained some level of shikai.
Kido: He is a proficient skill in Kido. Hanataro has been known to use spells as high as the mid sixties. Whether he could do it higher is currently unknown, but should be considered to have at least some skill in defending himself. Memorization: He was able to memorize the tunnels underneath Soul Society with ease, and has knowledge of where most supplies are kept. Once he knows a place or a technique, he is unlikely to forget.
Cleaning: I say this as he seems to have some ability to clean well, at least to the point of pleasing the nobility. He seems to know how to do regular house work. It would be likely that he would apply this to his medical abilities, as sterile environments are best for healing.
Sixth sense/Second sight/whatever may apply: Hanataro, as he is of the supernatural himself, is able to see ghosts, hollows, and other things clearly in the mundane world.

Personality: Weak. A hindrance. An eyesore. These are some of the words that may describe Hanataro at first sight. He is not a person that emits much in the way of confidence or competence. A person could even go as far as to call him a loser. He has cited that he's "the most bullied kid in the world." It does not help either that he is rather gullible, and an easy target for bullies. It's no wonder that Hanataro is as insecure as he is. In addition to this, his brother, Seinosuke Yamada, was once ranked Vice Captain. There could be additional troubles from that due in part to what is said his brother's bad character, and the power he wielded. It is unknown if Seinosuke was imprisoned or is even still alive at this point. But either way, Hanataro may feel that he has to become stronger than his brother ever was, or feels he has big shoes to fill. The bar is set high; it is possible that he will never be as strong in either prowess or will. It is little wonder that Hanataro is a quiet and humble young man. It is also little wonder that he harbors a potential inferiority complex.
Still, although he tends to be a nervous clumsy mess, Hanataro does have his strengths. He does not lash out against those who harm him or insult him; he scolds when he believes excessive force is used. He is flexible, intelligent, and will do whatever it takes to help his friends, even if it means enduring punishment. He does not tend to be the impulsive type if he can help it; instead, he thinks ahead, as to ascertain what would be best not for him, but for the entire group. If he thinks that a bad choice is being made, he'll call them on it. He has a quiet courage, which is subtle, and harder to notice. This courage was what allowed him to make peace with those that he believed to be his possible enemies-the very same that attempted to use him for a hostage, only to run away. It is the same courage that pushed him to the bridge, unarmed, to take on one of the most dangerous people in his world in order to buy time for his friend's escape. Hanataro is insightful, knowledgeable of his surroundings and quick to analyze the hand he's dealt with. He keeps his goals realistic and has the possibility for great growth as a person more assured of his worth to others.
Most likely he will continue to stutter and to flail in the near future. However, Hanataro has finally begun to realize that he is worthwhile and his opinions matter. With such on his mind, he has began to take himself a little less seriously, and started to work on his weaknesses-such as his stamina and ability to fight.
Hanataro does not just live to be a comedic relief, or to be relied upon. He has his own goals and ambitions before Ichigo came to rescue Rukia. He was the seventh seat of the fourth company. Though he often modest about his position, he could be considered the seventh most powerful healer at this time. It is unknown if in the seventeen month period if he became more powerful, and was promoted. It is possible. He is a very hard worker, and does not stop until he has completed his tasks.
Hanataro has been known to be diverted from his work ethics, onto other, more personal projects, however. For example, due to his clumsiness, he was taken by Ichigo as a possible hostage, and then toted along by accident. It looked as if he was going to leave or be left behind when Rukia's name was mentioned. And just like that, he was leading the way. No questions asked, for he had heard Ichigo's story when he was cleaning her cell. He knew that Ichigo was reliable in getting her out. And that's all he needed.
It could be argued that Hanataro has romantic ambitions in mind; that he feels for Rukia more than just mere friendship. That is entirely possible. However, he likely would not act upon his romantic feelings. Not because he feels shy; it is obvious in the flashbacks that he feels quite comfortable in her presence. However, he is also aware of Rukia's station, and the how likely the feelings would be returned. Hanataro may have assumed that her feelings were for Ichigo and him alone.
Romance may not have been enough to rescue Rukia though. He has been noted throughout the arc for being reliable to his new found friends. As well, it seemed that his own Captain was disappointed by what could have been entirely out of character for Hanataro-abandonment of duty to aid the enemy. Most likely, Hanataro thought of the crime that Rukia was facing, and of the scale of punishment. Like others within the Shinigami ranks, something seemed wrong. The punishment did not fit the crime, nor Rukia's personality. It's likely that Hanataro mentally chafed, and weighted his ethics of duty against going against what was morally wrong. To send a good woman to her death, without a trial or even a lesser sentence would not sit right with Hanataro.
This does not mean that the healer meant to rescue her by himself. The system was against him, and his absence would have been noted. If he had not been taken, likely Hanataro would have continued to kick himself, and feel guilty for doing nothing. However, when the opportunity came to set things right (even by going against the Gotei and facing punishment for doing so) he took it. If the system was wrong, then he was going against it. No doubt if Hanataro had been presented the same opportunity, aware of the repercussions of his acts, he would have taken it. He would not want anyone harmed for his actions, far from it. But to do something would be better than doing nothing. Taking action would be better then ignoring what was morally wrong.
