I think I've seen others prompt this before, I've never seen a fill, so:

Sherlock is attracted to John. John is most certainly attracted to Sherlock. So, why the hell haven't they shagged yet?!

John kisses him, John holds his hand, John cuddles with him on the sofa...and then, John politely excuses himself to the bathroom to either attempt to calm himself down or, when that fails, masturbate. Oh... Yep, that makes sense. He can't really fault John for his assumption. He did, after all, tell him that he was married to his work and it's not as if Sherlock's been with anyone since they moved in together.

And, upon closer inspection, Sherlock can see the thought that he'll be Sherlock's first obviously means a lot to John...and really turns John on
