I recently made a bell-shaped petticoat from a pattern from Otome no Sewing Vol. 6. It's four layers of tulle and has a pretty nice cupcake shape. The outer layer is basically the way an A-line petti would work with four strips and measurements similar to what you've described, but the three inner layers are two thin strips sewn together (40 by 19 cm for two of the layers, 40 by 13 cm for the other) with two gathered longer strips (again sewn together) underneath (90 by 33 cm for two layers, and 130 by 39 cm for the other). So, the inner two-partlayers create a subtle cupcake shape and the outer layer adds most of the poof and drapes around the inner layers (esp. because it's a little longer than the other layers.
