Dean was just past the first trimester, but he looked more likely to be closer to full term with twins or triplets. Dean was constantly hungry and also complaining about everything and anything. Sam didn't complain and tried to meet Dean's cravings, even if he changed them in the middle. He was also happy that they had the twins, as then he knew some of Dean's expressions when things were coming.
Even if Dean was large, he had got his sex lust back, which a few weeks ago he thought he had lost. That meant Sam had to draw his part. There were a few positions that seem to work. One was with Sam lying on his back with Dean sitting on him, with the belly touching his stomach. Dean couldn't bend forward and kiss him, so he let his hands roam over the belly and he mouthed endearments. Another way was if Dean was on his back, at the edge of the bed, and Sam was on his knees. Quite soon they decided that they didn't like it as even here they weren't able to kiss. Not only that they couldn't kiss, but also that Dean often had a hard time keeping himself comfortable on all fours.
It was best when Dean lay on his side and Sam was behind him, as then he could booth stroke the belly and nuzzle Dean on his neck and get some quick kisses. Though to Dean's frustration they didn't make love enough times. They had to think of their twins and all the little ones Dean was carrying. To silence some of those complaints Sam gave some blow jobs and let Dean do that on him as well.
