survey language advice

I'm editing questions for a study questionnaire that will be administered via computer, over the internet. Many of the people participating will be teenagers, so we're trying to make it as straightforward as possible.  For medicines (like certain antibiotics), we're asking them whether they take it, and then, in separate questions, how strong each pill is and how many pills they take per day.

I'm drawing a blank on phrasing.  I had "What dosage are your pills?" followed by a dropdown box, but "dosage" really describes how much of a medicine someone takes in in total, not how much is in each pill.  And I need something standard because we're asking about lots of meds.

Anyone?  I don't have experience with writing questionnaires, but I feel like there's probably a science to phrasing questions. At this point I'm not sure I have enough distance from the thing to know if the phrasing is clear enough - and I'm editing it for someone else on a rush status.
