Honorifics question

I did want to ask "people in the know" questions about Ouran Host Club and the use of honorifics. (Research question!)

1. Do the Host Club refer to each other by their first names because they are all in the same friendship/group? I know that Tamaki gets called "Tono" by the twins, but that's more of a smartass gesture than anything.
2. What honorific (if any) do the boys use for Haruhi? Hunny always uses "Haru-chan" to denote his loli-shota tendencies, but I swear that the only other time I've heard anyone else use an honorific was one of the twins calling her "Haruhi-kun" in class. If they don't use honorifics for her, why do you think that is? (Commoner status, yobisute, etc etc etc.)
3. What level of politeness are they using in their speeches to each other? Renge calls Kyouya "-sama" for awhile to indicate her infatuation with him, but I notice that he calls her "Renge-kun" at some point. (Is -kun slipping into unisex usage these days?)

... I guess I was wondering because Ouran seems to be the sort of place where people would be extremely aware of honorifics, but I'm not perceiving them as much as I would in another anime.

It was interesting to read all the replies. I have a question, if it's alright. I think Nekozawa's sister referred to him as Oniichama in one episode. Is the -chama part a combination of -chan and -sama, or is it another ending that I've just never heard of?
