Set in season 4. The angels think Sam needs to be taught a lesson but they need him for their plans later and so don't want to kill him. They snatch him from a motel room and drop him in the middle of a desert to get himself out. I expect the usual dehydration, sunburn, hallucinations. You get bonus points for snake bite. It is up to you if Sam gets himself out or Dean comes to the rescue.
Dean is unconscious, Sam is badly injured but they are in an unstable location(cave-in, abandoned building, house fire) and Dean will die if he doesn't get him out. Sam, even though he is in agony, carries his brother out, killing the monster on the way. I just want really BAMF!Sam fighting to save his brother even though he should probably be hospitalized. If you want to take it further and have Sam's collapse, Dean's waking up fine, and Sam's journey to recovery, be my guest.
What if the trials really did purge Sam of demon blood? But what if the demon blood was giving him huge advantages in health growing up (height, immune system boost, etc.)? Now without it, even recovered from the Trials, his immune system is just not what it used to be, and he catches every little cold and flu going around. And maybe takes longer to recover from injuries as well.
honeylocusttree had this notion, similar to your prompt, that if the demon blood was responsible for Sam's height and girth, what happens when it's gone and Sam isn't the "moose" anymore? What would the mathlete be like? Many possibilities here!
I made this ten times more sappy than I originally intended too. I apologize for so much schmoopiness. It is un-beta'ed so all mistakes are my own.
“Sammy, stop scratching.”
Sam actually pouts, his hands awkwardly clenching and unclenching before settling awkwardly at his side. The rash adorning his chest is impressive. It spread like wildfire and no amount of oatmeal baths and patented socks tapped to his hands seems to help.
Given that Dean only has experience with Sam having the chicken pox; he really doesn’t have a clue what to do.
When Sam’s hands begin to shake at his side with the urge to scratch again, Dean sets down the newspaper he was reading and marches into their bathroom to look for anything that could possible help the itching.
Who knew hellhound blood was such a bitch? The irony that Sam is somehow allergic enough to get a rash this size isn’t lost on Dean, nor Sam despite how much Sam glares at him when he mentions it

FILLED: The Itch You Can't Scratch 2/2The cream must be helping because Sam seems to relax back into the chair. Once Dean rubbed a liberal amount over the rash he straightened up and read the bottle again

What if the Nazi Necromancer poison didn't go away nearly as quickly as it did in the actual ep? Or had some nasty aftereffects?

Sam gets a nasty rash from the hellhound blood all over him.
(Why no, this is not a shallow attempt to bait someone into writing fic where Dean rubs cream all over Sam's naked chest, why do you ask? ;)
Edit: Gen or slash welcome.
