"We have assigned a 70% evaluation for you major depressive disorder based on:
-Intermittent inability to perform activities of daily living
-Occupational and social impairment, with deficiencies in most areas, such as work, school, family relations, judgment, thinking or mood
-Impaired impulse control
-Inability to establish and maintain effective relationships
-Neglect of personal appearance and hygiene
-The examiner's assessment of your current mental functioning, which is partially reflected in you Global Assessment of Function score found below.
-Difficulty in establishing and maintaining effective work and social relationships
-Disturbances of motivation and mood
-Flattened affect
-Impaired judgment
-Impaired short- and long-term memory
-Depressed mood
-Mild memory loss
-Panic attacks (less than weekly)
-Suspiciousness
Your Global Assessment of Function (GAF) score is 55. A range of 51-60 indicates moderate symptoms; or any moderate diffculty in social, occupational, or school functioning."
Turns out I'm as fucked up in the head as I had always thought. What's worse is that the list above isn't even all inclusive...Meh.
