I've read posts from your journal from time to time, but other than the fact that we both went to BHS (which is how I stumbled on your journal) and have young children, we have no connection that I know of (though if you have two younger sisters, it's likely I might have known one or both of them at least tangentially).
On the other hand, I hate to leave a fellow traveler sitting stranded by the side of the road. So, I'll take a swing. Tell me to take a leap if you find this intrusive.
With both of our girls (3 and 7) we found the thing that helped keeping things going was encouraging a consistent schedule, e.g. "We're leaving for school in 15 minutes honey, time to go potty."

I think at this particular moment, it's a control issue. She's willing at day care; less so at home. To add to the weirdness, just today in the last hour and a half at day care, she had 3 accidents. I think we're going to back off it a bit, let her feel in control and unpressured, then in a few days start back up with hourly potty breaks while at home.
Wow. Three times in an hour and a half? Hmmm, that is odd. By control, I'm taking it you mean her control of the process rather than her control of her bladder. I'd say you're taking the right tack to back off a bit. With our younger girl, we got off to a fast start but then regressed back to plenty of accidents (I think she found it novel for a week or so, then got bored). We took it easy for a while and eventually she got back on track.
I will give you one bit of propaganda to look for in your local library. Once Upon A Potty on VHS. If you can find it, watch it once before you let your daughter see it. It's a trifle bizarre, but was something the younger one ased to watch over and over again.
