A bit of speculation that I have seen, is that it means Wilson goes both ways.  I actually was already thinking this could be possible because a couple of spoilers said House finds out who Wilson's secret companion is,but they never say secret woman.  Probably reaching

It's a location-filming site sign. The text on them is often mirrored like that. Here's another example:

But Wilson himself is not a set location. The reason they put the name upside down like that is so that they can flip them in case they want it to be to the left rather than to the right. The sign that GY posted is obviously not real, it's photoshopped onto a real sign in the middle of the street. Doesn't seem like the full picture.

Of course, it's not obvious... I just think the sign looks photoshopped and don't know why a set sign like that would be in the middle of a street, and looking like a normal sign! That would be a little peculiar. Unless you mean something else by that, hahaha. GY is a very strange man, anyway. Trying to decode half of what he says is a superhuman feat.
