No, you're not the only one struggling with this. You're mostly the same you that you've always been, you just have different responsibilities now. You've got the kid, and the husband. You've also got one of the craziest most stressful careers out there. So cut yourself some slack kiddo. =P
I'm not all that great at prioritizing time outside of work (but great at work, go fig). Not that you need one of those minute by minute schedules, but you could draw up something rough that would give you a set amount of time to do things (don't forget a few minutes for play =) ). You could always do a weekly or bi-weekly menu plan that would make it easy to shop for. That would also give you a chance to plan healthy choices for dinners and stuff as well as cross utilizing product (can you tell I'm in food slavery...). I've also heard that with this system it's nice to have one night that is like take out/dine out, and another (or two) that are favorites (mac-n-cheese, meatloaf, etc.).
As to cleaning stuff up and getting rid of it... I'm still working on this myself, but I've made a lot of progress. It's taken several years though most of mine was from the false feeling that I needed objects to show that I existed, to kind of validate me. Then there was the thought that if I acquired enough crap I'd never be able to move it all and then we'd have to stop moving every couple years.
Anywho, what I've found works well is to get a couple boxes, take stuff in one area and look at it and decide if you really need it. Is it something that really means something to you, something you use, or is it just junk. If it's junk then you can put it in a toss/give away box. If you're not really sure, put it in the other box. When you've filled the boxes (or finished an area) get rid of the toss box right away so you don't go digging in it (I've done this, it wastes time and brings back bad memories). As for the unsure box. Close it up and set it aside and leave it for a couple months. If you find yourself wondering where something is and you check the box and it's in there, then you need it. If you haven't needed anything in there in a couple months then get rid of the box.
As you acquire new things you should look at your old stuff and decide what to replace so you don't get cluttered up again. =)
Oh, and as some wise woman once told me, you don't have to get rid of all your stuff and live like a monk. =P
You could always try to get your friends to put you on What Not To Wear, then you could go to NY and meet Stacy and Clinton (love them) and get $5000 of free stuff. =)
