I ended up, after doing extensive research around the web, reading reviews etc., splurging and going for the Ultimate Ears 5c. They make them from a silicone mold of your ear, so they fit absolutely *perfectly*, and the fit means that you get great sound, and other sounds are really blocked out (also there's none of that annoying buzz to other people around you). So I listen on about half the volume I used to listen on the earbuds (I've been using the new ones for two days now). They're very expensive, but I figured it was better than buying one $50-$100 set after another and having them not fit (I've always had a tough time with in-the-ear earphones, because my ears are funny shaped and shallow). I figure it was an investment - I can also use them to listen to other things too, of course; I like to watch DVDs on planes, for example, and these are really good at blocking out all the external noise.

I guess Ultimate Ears has made this type of earphone for a long time for professional musicians; most of them are thousands of dollars, and they recently entered the "iPod market" with the 5c, which is their lowest priced model and geared toward amateurs - doesn't have all the extra crap that real audiophiles and professionals need.
