They never were friends, not really. Ino hoped that somehow she might get out of being part of the next InoShikaChou trio and be with cooler people. Shikamaru and Chouji were acquaintances, and while they enjoyed each other's company, both were too laid back to make a serious effort at friendship.
On Ino's tenth birthday - which was the day after Shikamaru's incidentally - Inoichi threw a party for the three of them. Ino pouted about not having Sasuke-kun or her friends attend, but she was quickly overruled by her parents. Shikamaru was dragged out of bed early and Chouji bribed with food to make both agreed to just follow. This was tradition, the trio was told. None of them thought to ask what the tradition would mean.
Later that night, when the drinks were nearly finished, Inoichi pulled out a tray of ice cubes, and Chouza produced some needles as Shikaku lit a match to make sure they were sterilized. The kids squirmed and protested, but their parents remained firm. This was their right of passage. One by one - first Ino, then Chouji and finally Shikamaru - their ears were pierced with small hoops.
It was a promise for the future.
