I can't think of a subject right now for there are many...

Thursday...  So I had my oral exam.  I came in at 8:30.  Well a little early.  I find Lucretia / Cyril.  They had their exam the day before.  I asked them where I go, etc.  Cyril was like he'll come find you.  OK...  Soon Grace?  Comes running up to me saying they wanted me in L&D at 8:45.  Dr. Rothschild is asking where the hell everyone is.  Then I need to make photocopies of my list.  OK...  So I do that.  Give him my list.  He's like I'll find you in clinic later.  OK.  So I wait...  I've been hearing people either do crappy / great.  Like they give you several topics, you end up doing half and half so...  I was feeling somewhat OK, plus they're not mean.  Very patient.

God...  I'm so sleepy I feel lightheaded...

Anyway, where was I?  So pretty much I get called.  When I first brought in my list, he's like only 13 patients!?!  So I come in.  He's like short, but interesting list.  So my 13 patients had...
  • Hypothalamic amenorrhea
  • Polycystic Ovarian syndrome
  • Fibroids / Myoectomy
  • Preeclampsia
  • Labor Induction / C-section, BTL
  • Polyhydramnios
  • Mild to Severe Preeclampsia
  • Open Cervix / dexa tx
  • Fetal Demise
  • IOL 2ndary GDMA-2
  • Inevitable Abortion
  • Twins
  • IOL 2ndary to seizure disorder.

First he asks me if the patient had primary or secondary amenorrhea.  Primary.  Describe it.  He was like huh?  I said no menstruation by 14 or if 2ndary sexual characteristics develop, by 16.  He's like rephrase.  How???  Anyway, it was like if you don't get your period by 16, period or if 2ndary sexual characteristics develop 2 years after and no menses.  Then he's like give me reasons for primary.  Uh...  Pregnancy.  In this day and age, yes.  Chromosomal abnormalities.  Turners, uh...  He ended up naming a lot others.  What was her cause?  I don't think he ever stated / they knew.  She had an MRI and they had no findings.  She was on OCs which made her bleed.  Then asked me about Preeclampsia.  Differences between mild and severe, symptoms, treatment, etc, etc.  I got that.  Polyhydramnios.  Ugh...  Causes.  I said renal first.  Then GI.  GI abnormities that would cause it.  Atresia?  Out pouching?  What else?  I don't know...  Abortions.  Definitions of each.  What you'd find on physical exam.  I was like maybe hemodynamic instability.  They were like Gyn...  OK, uh...  open cervix?  Twins.  They just wanted to know how they were delivered.  One was vertex other breech so they c-sectioned.  I think I did...  Like others.  Great / crappy.  Got a written exam Monday.  Not worried.

Oh yeah, I was off on my b-day right.  I came in Thursday.  Crap...  We had Grand Rounds...  Was I supposed to come in for that.  I wasn't the only one out though.  No one noticed either.  But there is a sign in sheet...

Today, I got out early.  I get a phone call from Lucretia...  Dr. Rothschild had a meeting.  He wants to see you (and Cyril and Chidebe) on Monday.  She doesn't think we're in trouble.  Sure...  Hey, Dr. Zhang excused me.  My regular lady isn't in.  Not like we do much anyway.

Clinic today.  Well.  Monday, I was following that nurse practitioner.  Tuesday I was in high risk.  A lot of Diabetics really.  Thursday actual diabetic clinic.  It was Dr. Akpan and Dr. Zhang.  Dr. A would have to present to Zhang, but she kept going back and forth asking the patient more questions / looking up things.  I admit...  I'll probably do the same.  But...  My God it was annoying.  Then...  I mean, she looks like she's clueless but she's really very good.  I mean, I'd be all it's routine, but she's like getting the little things.  I'm afraid I'd miss that.  Ew!!!  Then Dr. Zhang comes in and gives her an extra pastry she had.  She's like send the medical student to get you tea or something.  Uh...  No.  I mean, had she asked I would, but, I'm not gonna offer.  My job as student isn't to serve you outside of medical education.

Today I was with Dr. Zhang.  Even though I should be in Ob clinic, I was in gyn.  Ever time a person needed a pap smear, I was dreading it.  She let me do one.  OMG...  I was fumbling.  I didn't know how to hold things.  Seeing it is way different from doing.  Then I had to do a bimanual exam.  I didn't feel anything!  Nor know what I was supposed to be feeling.  I suck!

Preggercy wheel...  I realized if I get pregnant on the 5th, I'd have my baby in perfect timing!  I'd have like a month off before I start residency, etc, etc.  Now...  Who will I get to impregnate me?

I heard the funniest overhead announcement...  Bake sale in the lobby...  All proceeds go to the Diabetic walk.  LOL.

I then had to make an overhead announcement...  Would the provider with the transvaginal US please bring it to the clean utility room.  Thank you.  Lucretia was laughing...  Cyril said I sounded very professional.

Anyway, we needed the trans vag for this woman who basically had a spontaneous abortion.  Check to see if she had actually passed it yet.  As far as she knew she had not, but was bleeding heavily.  They were giving her options.  Do nothing and she'll pass it on her own.  Medication, which they can't do cause legally they can't.  But...  Why not?  The baby is already dead.  Or surgery, a D&C.  She ended up getting the D&C cause the US showed retained stuff.

    Moving on...

Where did I get this???  Anyway, I got it off some newsletter I get.  Constantine Maroulis will be in The Wedding Singer for 8 weeks.  I wonder if Zach will visit.  I mean...  It's got Constantine and David Josefsberg (the original Abe).  Wednesdays???
