I use Photoshop CS4, but this Tutorial is easily translatable to lower versions.

By the end of this tutorial you should know how to get from this:

Step 1: The Base
The first thing you need to do is finding a suitable base cap. I got mine from
here.
I used these three pictures:

Step 2: Blending
The original size of each HD cap is 1280x720. Of course this is way too big which is why we need to resize them first. For this, I open a new file 450x150 (this is the size of the blend).
--> Now go to image > Image Size and change Height: 720 to Height: 150 on each of the three base caps.
--> Next, you copy&paste each cap on the blank 450x150 layer.
Your image should look like this:
The blending is pretty easy. You need to keep in mind that Cap2 is on top of Cap 1, and Cap 3 is on top of Cap 2, okay?
--> Use the Eraser Tool, using Airbrush Soft Round 45px (size doesn't really matter, as it depends on the size of your base).
--> Make sure that you are working on Cap 2! (it's probably called 'Layer 2')
--> Carefully erase the part of Cap 2 which overlaps Cap 1
--> Do the same on Cap 3, until you're satisfied with the result.
Your image should look like this:
Step 3: Coloring
Now that we have the base image, we can start with the Coloring. First though, you'll need to merge the three base layers, in order to make it easier to work on them.
--> Open a new layer (Layer > New > Layer)
--> Go to Image > Apply Image
--> Duplicate that layer and set it to Screen - opacity: 77%
--> Go to Layer > New Adjustment Layer > Color Balance
Midtones: +16 | +33 | -33
Shadows: +17 | -7 | -11
Highlights: +30 | -7 | -28
Here's what you get:
--> Go to Layer > New Adjustment Layer > Selective Color
Reds: Cyan (-19),
Magenta (+5),
Yellow (+16),
Black (+7)
Here's what you get:
--> Go to Layer > New Adjustment Layer > Selective Color
Reds: Cyan (-5),
Magenta (+5),
Yellow (+38),
Black (+16)
Yellows: Cyan (-10)
Magenta (+5)
Black (+100)
Whites: Black (+22)
Neutrals: Cyan (+23)
Magenta (+12)
Yellow (+7)
Blacks: Black (+23)
Now it should look like this:
Step 4: Texture & Text
For this next Step I used this Texture (I honestly don't know where I got it from - if anybody knows, please tell me, so I can credit properly).
The original size was 800x533. I resized it to fit my picture (450x300)
--> Copy&Paste the Texture on top of Alastair
--> Set it to Screen and move it if necessary, until the effect satisfies you.

Now on to adding the Text!
I used the wonderful Font "El&font Gohtic", which you can find
here --> Write the text wherever you want and make sure to color it white.
--> Set the Text-Layer to Soft Light, full opacity
--> Double click on the Text Layer and check the box "Drop Shadow"
--> In order to adjust Setting, click on the "Drop Shadow" bar and use the following settings:
Opacity: 100%, Black
Angle: 120°
Distance: 4px
Spread: 0%
Size: 2px
And this is it, you're done! This is what the end-product looks like:
