harry potter/bleach crossover fic find?

I've been looking for this for some time. so I thought I'd try my luck here :)
A Harry Potter/Bleach crossover, harry/draco pairing.
Harry and Draco die and mistakenly end up in the bleach universe, meeting up with the other shinigami's, where they train as them. As years pass, they find thier zampaktu's. Harry's is a stallion and Draco's is a dragonfly, it special ability is to make him disappear. Draco pined for Harry but had a short love interest with a side characted named Hajime, but in the end of course, ends up with harry.
At the end, they are found and go back to the heaven they were supposed to go.
It was a very well written fic. :(
