He's not sure what's wrong, but, Dean being Dean, he tries to keep it to himself. Until it lands him in the ER, where the situation goes from bad to worse.

Reccer’s comments: It's a pilot!AU, wherein Dean goes missing and John goes to get Sam from Stanford. I have never seen this one recced here, why not I really don't know because it's gorgeous. So much hurt!Dean (with a generous side-order of hurt!Sam too, but mainly Dean), and traumatised!Dean, and protectiveness and... yeah, I could keep going for a while. The link is to the first fic in the verse, but if you go to the author's journal there is a load of tag-fics and they have just started posting a new multi-chapter in the verse, which gets updated on Sundays.
