Bilbo returns to Erebor

So glad to have found this community.

I'm looking for a fic that I read a while ago, pretty sure it was completed and on ao3.  Bilbo goes back to the shire for some reason, but returns to Erebor for ......reasons? (I think with a young Frodo, but I may not be right on that bit).

After he returns to Erebor, Bilbo gets really tired and becomes sick, but after he is sick he becomes younger, and has his life extended to that of a dwarf's so he can live out the rest of his life with Thorin.  He also gains the ability to have children.  All this is explained in a dream that he has of Yavanna and Mahal while on his way back to Erebor.

Any help in this would be greatly appreciated.
