revisions, accounts payables... taxes in canada

1. i'm looking at RGD ontario's proposal forms and they don't say anything about covering my ass when the client makes me go above and beyond. do you guys have overtime/extra services clauses? what do they say? what is your timeframe for revisions?

i was thinking of saying something like The quote provided is an estimate calculated using the above terms. In the case that the project exceeds the timeframe, number of revisions, or services listed above, an hourly rate of $XX.XX will be charged.

is that even necessary? i don't know. i want to make sure i have standards and clients are aware of them, but i'm not sure when/where is appropriate to state these things.

2. i see billings does accounts receivables, but what about accounts payables? am i stuck with quickbooks?

3. canadians: of course GST is mandatory. what do you charge PST for? (the RGD site just says the government doesn't set a standard for graphic designers.)
