Changing Social Security Card/Birth Certificate
Just some info for those who live in Allegheny county (PA) you will need to change your birth certificate BEFORE you can change your SS Card.
You will need to to fill out & notarize the corrections form for your birth certificate (notary $5.00) AND you will need to fill out an application for a new birth certificate. You will need to included the certified copy of your name change, surgery letter, copy of drivers license, check for $10.00. This will take 8-10 weeks.
Once you receive your new BC take it along with your drivers license & an application for a new SS card to the SS office. It will take 2-4 weeks for a new ss card.
You cannot change your SS card without changing the birth certificate first! I tried & it didn't work! lol
