PIP part 4

Feb 24, 2017 18:37

As you know I (a UK citizen) am in the process of claiming the crip benefit called the Personal Independence Payment (PIP)/.

Since my last blog post about PIP I've made progress with the infamous 40-age form. I'm making rough notes - very rough with my hands the way they are. For the form's first real section 'About your health professionals' I responded to Q1 (details of my doctors and nurses) by giving info about my general practitioner (GP) and for my consultant neurologist, also about the orthopaedic surgeon who put the pot (plaster) that now is on my leg. Then info about the specialist nurses who work with my neuro.

When I see the ortho a few days from now, I'll find out his postal address and his phone number. For the specialist nurses I phoned their number and asked where letters to them can be sent.

Next, the PIP form asked about 'other conditions', how long I've had those and what I do for them. I retrieved that info from the UK MS Register and from my repeat prescription.

I've made more rough notes for the PIP form. Will tell you about those but now I want a break.
