PIP part 3

As you know I (a UK citizen) am in the process of claiming the crip benefit called the Personal Independence Payment (PIP)/.

I now face the form 40-page form "How your disability affects you". It comes with a 12-page Information Booklet. I'm an academic so 52 pages of subheadings didn't scare me off but for others, this could be a real barrier.

Of course I'd to check my name and National Insurance (NI) number. For some reason we in Britain don't simply use the NI number as other people use an identification (ID) number.

The form's first real section 'About your health professionals' began with Q1 asking for details of my doctors and nurses. For my general practitioner (GP) that info's easy to find. Not so easy for my consultant neurologist but I found it in my Medic Alert paperwork.
