Horses are dangerous. Asking horses to train for years to over come their rational fears of jumping AND putting a monkey on their back. Dangerous. Have I done this, yep. Fortunately for me I have not taken terrible spills costing any loss of life. The majority of my injuries over the years have been on the ground where I'm NOT paying enough attention.
This is an excellent piece about the danger, some of the reasons why, however one thing it has not covered is the breeding choices that have weakend the structure of the present day generation. I own a TB mare that is on permanant pasture, she is weak in mind and body and WILL cause injury. My best TB was very sound and saved my life on one occasion. Accidents will happen. Forcing the increase fatality of horses and riders IMhO is the IOC and people rushing a learning process that a human isn't completely trained for 20 years.
