Using Adobe Photoshop CS3.
25 Steps.

Number one. Crop your base to 100 x 100.
Number two. Duplicate your base and set to Screen at 100% 
Number three. Duplicate the screen layer and set at 100%
Number four. Duplicate your base and drag it to the top. Set to Soft Light at 25%
Number five. Create a new layer and fill with 06152c and set to Exclusion at 100%
Number six. Create a new layer and fill with ff3000 and set to Overlay at 6%
Number seven. Create a new layer and fill with 977033 and set to Multiply at 35%
Number eight. Create a Color Balance layer -

Midtones: -60 -15 +12
Shadows: +20 +3 +3
Highlights: -19 -2 +25

Number nine. Create a Selective Color layer and set to Normal at 50% -

Reds: -100 +61 +100 +47
Yellows: -100 +100 +100 -40
Cyans: +100 +100 +100 +0
Whites: +100 +0 -36 -53

Number ten. Create another Selective Color layer and set to Normal at 25% -

Reds: +12 +24 +37 +14
Yellows: -100 +75 +100 +0
Whites: +0 +0 +21 +0

Number eleven. Create a new layer and fill with 06152c and set to Exclusion at 50%
Number twelve. Create a new layer and fill with 977033 and set to Multiply at 15% - 
Go to Image - Apply Image - Blending: Screen

Number thirteen. Create a new layer and fill with ffdbfb and set to Soft Light at 95%
Number fourteen. Create a new layer and fill with c6ceed and set to Color Burn at 11%
Number fifteen. Create a new layer and fill with 919191 and set to Color Burn at 15%
Number sixteen. Create a Curves layer and set to Normal at 36% -

RGB: Input: 212 Output: 195
2nd Point: Input: 48 Output: 35

Red: Input: 207 Output: 188

Green: Input: 152 Output: 155
2nd Point: Input: 68 Output: 68

Blue: Input: 190 Output: 179
2nd Point: Input: 65 Output: 66

Number seventeen. Create a Selective Color layer ans set to Normal at 20% -

Reds: -100 +38 +100 +0
Yellows: -100 +27 -2 +0
Whites: +100 -15 -37 +1

Number eighteen. Create a new layer and fill with e5e5e5 and set to Darken at 100%
Number nineteen. Create a new layer and fill with d5965e and set to Multiply at 27% 
Number twenty: Create a new layer and fill with 08123a and set to Exclusion at 10%
Number twenty one. Create a new layer and fill with 463f43 and set to Darken at 6%
Number twenty two. Create a Hue/Saturation layer -

Master: Saturation: +2

Number twenty three. Create a Selective Color layer and set to Normal at 10% -

Reds: +100 +31 +28 +11
Yellows: -6 +16 -25 -1
Cyans: +100 +100 +100 -100
Blues: -100 -100 +100 +100
Magentas: -15 +0 +0 +0
Whites: +36 -31 +33 -19
Neutrals: +8 -3 +4 +4

Number twenty four. Duplicate that Selective Color (number twenty three) and set to Normal at 15%
Number Twenty five. Flatten image and Sharpen at 50%

You're DONE!

*Remember, you may need to change different layers. You may also need to add more screening layers at the beginning. The last step is really up to you. I usually like when my image has some sharpen in it.
