Healer Hermione

I'm looking for a very old fic where Hermione is a healer at St. Mungo's and her patient is Draco who is a patient in the Janus Thickey Ward. Draco seems to be stuck in his mind, he has out burst has moments where he is normal and talk to Hermione. They end up falling in love. Hermione figures out that to safe Draco she has to obliviate him. It ends with Hermione having obliviated him and walking away from being a healer and Draco. And then one day he apparates to The Burrow.

Does anyone remember this fic? Or the author?
