I like to think that after an initial period of shock, that people who love the Doctor would be able to recognize his innate core personality. I mean, I love the Doctor, and it doesn't matter to me which one it is. :)
Although I agree that the Doctor is the Doctor, I think that kind of recongition would depend on the kind of shock. For example it probably took Peri a minute or two to reconcile that the man who had knocked her the floor and was strangling the life out of her was the same man who hours beore had had struggled to milk a tiny bat, and died to gave her the last bit of the antedote. Same man--she watched him change--but it would difficult to prove under those circumstances that change of face did mean some significant change in the individual. Alhtough I do wonder if part of his unbalance--well not to the point that he strangles the woman--was due to what must have seemed Peri's rejection of him. River immediately recognized the Ten as the Doctor, but not necessarily the man she fallen in love with. She said he was young--and interestingly enough as she had seen his other incarnations in some form--younger than she had ever seen him, and thought his 'youth" made him difficult to deal with.
