So. Sam's sister today let us know results from the medical examiner on his COD. Slightly edited to remove personal stuff:

"1) died peacefully in bed, with covers up in sleep. M.E. was asked for evidence of a final "oh shit I'm dying" moment, but she said no. No evidence of any final moments of pain or struggle.

2) Toxicology reports showed that there wasn't evidence of any fatal chemicals in blood or body. There was liquid, most likely a beer from earlier, in the stomach but no deadly toxins. Also no blood clots in the brain that she could see, no big arterial clogs.

3) Even when the heart sample tests come back, they might not tell us much. Heart failure or attacks are only registered while happening and unless there's clear evidence like clogged arteries, the M.E. can't actually "see" one in a deceased person.

4) Based on the chest pain mentioned having earlier in the night, and the fact that there was nothing to indicate this was done intentionally, it seems most plausible, yet entirely unfortunate, to the M.E. that the heart or the wiring just gave out. Hence, the additional heart sample tests.
