I've heard (and used) the "when" joke, but I'd never heard the joke on the album cover. I had to google it and check 3-4 links (after failing to "hear" it while attempting it -- it just sounded to me like I was mangling it, but not in any specific way) before I found what the "punchline" was.

Pinch your tongue between your thumb and forefinger and say "I was born on a pirate ship." It will then sound like something else that would be amusing to a 12-year-old boy.

We were always taught to pull your lips apart like the boy in the photo. As some random poster points out below, one can optionally add "with a bunch of apples"

Random thought...I was listening to "All I want for Christmas is my two front teeth" and realized how much the singer sounds like you. Be happy you have all your front teeth, so you don't wistle when you say "Sister Suzie sitting on a thistle".

Pulling your lips apart like the boy makes the phrase "I was born on a pirate ship" sound like "I was born on a pile of s#%t"...at least, it does if you're twelve.
