And Sam does rip Lucifer's heart out. He's rending and tearing at his own insides trying to get free, trying to save Dean, and that just makes Lucifer even more angry, but not angry enough to stop Sam from being witness to his brother's destruction in the name of love.
It's a risk that Lucifer is more than willing to take in order to break Sam of his tie to Dean and make him his forever. For all that Lucifer has been watching Sam, I dunno about you guys, but if he thinks that's gonna work, he sure doesn't know Sam and Dean very well, does he?
In Born Under A Bad Sign, Dean never considered killing Sam because he knew it wasn't Sam doing all of those things. He had faith that Sam was still in there and would be able to defeat this thing, and if he didn't, then he was going to die with him instead. Dean has always had faith... in Sam. It's been shaken in the same ways that his faith in God has been shaken, but it ultimately always returns... because he loves him.
