I feel like it's a combination of two delusions. There are people who are so seriously in love with fictional characters that they believe they have met/married them on an astral plane. See Snapes on a plane:

Then there are the furries, obviously, and their lust for anthropomorphic animals. I am always afraid that they are just inches away from abusing an animal for real. Ick!

Obviously, this guy's not full-on fetishizing Twilight Sparkle. He must have some measure of respect for her as a pony or he wouldn't get so upset about porny pony art. He seriously thinks that they are in a committed relationship and is hurt by people demeaning his fiancee.
