Two questions... for option one, do I have to use the provided images, and for option two does it have to not be 100x100?

You do not have to use the provided images for option one nor do icons of provided images have to be non 100x100. As long as your icons match at least one of the options, it'll be accepted.

And is text allowed?

No there was no violation of rules. For some reason, I thought that there were only 15 icons...and so when I put up the voting, as long as there were 15 icons and none of them were duplicated, I thought I was right.

I'm extremely sorry about that. I was not paying close attention.
