In the strictest sense, anything that burns is converting matter to energy, although at an outstandingly inefficient rate, so we'd have to include chemical explosives and firebombs.

No, you don't lose mass when burning or exploding things; you're just messing with the chemical composition of the stuff being burned. Usually, you're just adding oxygen.

No, John's right. There's a ridiculously small amount of mass lost in any exothermic reaction.
