At first, Alphonse hadn't even entirely noticed that pain.
It was a dull ache in his abdominal region, a stinging sensation that wasn't entirely unlike what he had been continuously experiencing lately, and what he was more or less used to, by then. It wasn't any cause for alarm, at first, as he was just sitting in his room, quietly, to himself, waiting for the coming of night. He preferred to be in his room when the sun was setting; it seemed safest that way. The room was a comfort, he didn't have to be out in the open, or anything, when monsters may or may not come out.
But this pain was different.
It was distinctly different from the other pains he had been feeling (especially as those had been centred on his joints - all of the many - and this was not), and it wasn't fading, and it was growing gradually sharper and more painful, and strange pungent scent coming along with it. He let out a yelp as he began to realise exactly how much it was hurting, a horrible piercing pain in his stomach region, and beginning to force its way out. That wasn't something that was supposed to happen; it wasn't something he had been prepared for. The pain was quickly growing blindingly (no pun intended) unbearable, and he felt the skin and muscle of his weak stomach beginning to shift, contort - to tear.
His voice died into a whimper, before he yelped again as there was a tear and a crack, and he toppled over, onto himself, and into a ball, unseeing eyelids clenching shut underneath the bandages, body curling tight and arms wrapping firmly around his stomach, as if trying to stop it from tearing open, as if trying to stop whatever it was from escaping - he felt like his organs were going to spill out, so he just kept gripping tightly, trying to prevent the pain, trying to prevent the horrible outcome. The stench of rot and decay hit his nose again, the pain nearly unbearable as he whimpered and whined. He wasn't even sure if it was over yet...
